    Procedure to Obtain a Water and/or Sewer Service Agreement

    In order to obtain a water and/or sewer service agreement from the Miami-Dade Water and Sewer Department (WASD), you must submit the following documents and information to WASD prior to preparation of the service agreement: 

    • A request in writing for the water and/or sewer service agreement from the property owner, his or her agent, a contractor, OR an engineer.  This request can either be dropped off in person at our New Business Office at 3575 S. LeJeune Road (south of US-1), or it can be mailed to us at: Miami-Dade Water and Sewer Department, 3071 SW 38th Avenue, LeJeune Bldg., Miami, FL 33146-1520, Attn: New Business Office

    Property Owner's information:

    • Property Owner's Name
      • If a corporation, must include corporation's name and state of incorporation
      • If a partnership, must include partnership's name and state where partnership was legally formedIf a Limited Liability Company (LLC), must include LLC's name, state where LLC was legally formed and the name of a Managing Member
        • Mailing address
        • Daytime Phone Number
        • Property Information
           
    • Two (2) original property boundary surveys, signed and sealed by by either a Professional Land Surveyor (P.L.S.) or a Professional Surveyor and Mapper (P.S.M.); OR Tentative Plats identified as "Boundary Survey," including location sketch at 1"=300' scale
       
    • The legal description of the property typed on 8 ½" by 11" paper

    • Location Sketch prepared on this template

    • The folio number(s) of the property (this can be obtained by searching the County's property records at on the Property Appraiser's website
       
    • Type and/or number of units to be constructed Schedule of Daily Rated Gallonage (PDF) 
       
    • Construction schedule, showing number of units to be completed per calendar year
       
    • Three (3) copies of the proposed site plan and tentative plat showing the layout of buildings and roads
       
    • If available, the proposed plan of water and/or sewer connection and layout
       
    • The property's current  and proposed zoning designation; the zoning designation MUST allow the proposed development
       
    • Proof of any existing or previous connected structure, and the type or usage/occupancy of said structure
       
    • A sewage flow allocation letter from the Miami-Dade Department of Environmental Resources Management (DERM) or a septic tank approval letter from the Miami-Dade Department of Health (DOH)
       
    • Payment of the appropriate fee as follows, cash or check ONLY in US funds
      • Water-Only Service Agreement Fee (If the proposed project is in a water-only service area) - $100.00
      • Sewer-Only Service Agreement Fee (If the proposed project is in a sewer-only service area) - $100.00
      • Water And Sewer Service Agreement Fee  (If the proposed project is in a water AND sewer service area) - $200.00

    Please note that at the moment the agreement is executed, you must also present an Opinion of Title as evidence of the Developer's ownership of the property.  Also, please note that the Service Agreement will be recorded by the Clerk of Courts when it is executed; you are responsible for the recording fees.  The exact amount of the recording fee is determined by the number of pages the agreement contains.  This amount will be calculated when the agreement is offered. 

    Additional information may be required based on the project location and size of the water and sewer facilities to be installed.
