    Please complete both forms below.  Forms must be brought with the student to class on the first day of training.  The Informed Consent, Waiver and Release form MUST be notarized.  Any student who does not bring both forms, including a notarized waiver form, may be subject to rescheduling and will not be able to participate in CERT training.
